Oh, that's completely weird.
I'd say, go for a software repair. A software repair will reinstall the software of your phone completely. It would help us to rule out the problem due to software glitches etc.
Make sure you back up all the important stuff on your PC, messages, pics , songs everything.
Once you're done with the software reinstallation, format your SD card once again and just put songs and pictures on it. See for how long it works without any problem. If it doesn't work well then you will have to get your phone examined at sony service center

Update / Repair your phone via PC Companion
Download PC Companion and install it. Do not connect your phone until you're asked to do so.
Launch PC Companion > Start Support zone > Start Phone/ Tablet Software update > select Repair my Phone / Tablet > Follow the on-screen instructions it shows.

Did you try to use another SD card to see if the problem existed ?
Does that SD card work in other phones ?
Try to format the card and then insert it in your phone.
Although it sounds like there might be a hardware problem related to SD slot.
